Our priority is to make you feel comfortable and welcome during your visit. We offer high-quality care and consultation for your health needs, while treating you with respect for your time and schedule.
We strive to understand any underlying health concerns that may affect you in the future, allowing your doctor to connect the dots and create a comprehensive plan for your wellbeing. We recommend starting with an annual physical exam, which enables us to establish a complete database of your health and create a sensible plan for your future.
For your first visit, we ask that you bring a copy of your last labs and any pertinent records, including a copy of any recent scans or immunization records. You can also bring your medicine bottles or take photos of them with your phone, and any home blood pressure readings.
Additional tests and scans can be arranged by our office, though please be aware that some screening tests may not be covered by your insurance. We encourage you to invest in your health today and prioritize continuity of care.
Please bring your insurance card and be prepared to pay any co-pays or deductibles required by your insurance. We ask that you arrive at least 15 minutes prior to your appointment.
Our goal is for you to feel comfortable and well cared for. At your first visit we will set you up for our Patient Portal. Once you are on our portal you can update demographics, pay bills, view lab results, request refills and send a message to your provider via the patient portal.
We work in teams and if for some reason your doctor is on vacation or not available we do have mid level providers and office in Frisco and one of our other providers can help take care of you.
What to Bring to Your Appointment:
- All medications, prescriptions and supplements you are currently taking.
- Past medical records.
- A list of current and past medical conditions.
- Your health insurance card, and driver's license.
