TELEVISITS
We will call you and schedule this with your provider of choice. This will be via video conference. We can go over your labs, refill your medicine, and discuss your health concerns. Most, if not all, insurances are covering Telemedicine for acute issues (not physical exams).
Once a date and time is confirmed, you’ll receive a a few minutes prior to your visit with a code to use and instructions to do a tele visit with us.

Telemedicine Visit Consent
Dear Patient,
We are pleased to offer you the convenience of telemedicine services through secure interactive videoconferencing technology. As a valued patient, it is important that you understand the nature of these services and your rights as a participant.
Please be advised that telemedicine visits are subject to the same standard of care as in-person visits and that, while not physically in the same room as your healthcare provider, you will be notified and your consent obtained if anyone else is present in the room during your visit. While we strive to ensure seamless technology, it is important to be aware of the potential risks involved, such as service interruptions, interception, and technical difficulties. Should it be deemed that the videoconferencing equipment and/or connection is inadequate, the telemedicine visit may be discontinued.
We also want to emphasize that you have the right to refuse or discontinue participating in a telemedicine visit at any time, without it affecting your future care or treatment. Your privacy and confidentiality are protected in accordance with applicable laws, and you may view the Notice of Privacy Practices or request a copy at any time.
Please note that your health information may be shared with individuals involved in scheduling and billing, and that your insurance carrier may have access to your medical records for quality review and audit purposes. Additionally, you will be responsible for any out-of-pocket costs that may apply to your telemedicine visit, and it is important to be aware that payment policies may differ from those for in-person visits.
By scheduling this telemedicine appointment, you attest to having personally read this information, having had any questions answered to your satisfaction, and fully understanding and agreeing to its contents. You also confirm that you are located in the state of Texas and will be in Texas during your telemedicine visit(s). This document will become a part of your medical record.
